established in 2007 as an LCC outreach program following a long history of community nursing in Lyme

provides FREE care for all Lyme residents both at the church and through home visits

have mobile medical equipment available to lend, including shower chairs, walkers, crutches, commodes, adult pull-ups, and more

provides a safety net to clients and their families in coordination with other town and healthcare resources

works with clients to understand often complex choices so that decisions are aligned with client goals and wishes

LCC's program has inspired the growth of parish nursing throughout the Upper Valley of Vermont and New Hampshire
